use this form to submit votes for Player of the Year following each match.
Player of the Year votes will be submitted to the fixture coordinator.
The Player of the Year dinner will be held at a time & venue to be announced.
You will receive a copy of this vote via email.
Please enter the jersey numbers of three players from each team to nominate for Player of
the Year. Players from your own team will only be included if the opposition
fails to submit
a Player of the Year nomination for the match.
